To provide clarity regarding our way of working and to optimize the cooperation between you as a patient and PC2 Tandartsen as your healthcare provider, we have established a number of house rules. We kindly ask you to read the following house rules carefully.
Treatments are provided by appointment only. We also offer second opinions upon request.
If you are unable to attend your appointment, we kindly ask you to notify us at least 24 hours (one working day) in advance. This allows us to offer the appointment slot to another patient. If you fail to cancel your appointment, or do not cancel it in time, the reserved time may be charged. The practitioner will determine the amount reasonably, based on the planned treatment. In the event of repeated missed appointments without notice, we reserve the right to remove you from our patient register.
If your personal details change, we kindly ask you to inform us as soon as possible so we can update your file (for example: a new phone number, email address, home address, or changes in your general health).
We are bound by professional confidentiality obligations. This means that we may not share information about you with persons or organizations that are not involved in your treatment.
All necessary information required for patient treatment is recorded and stored in accordance with European privacy legislation, the General Data Protection Regulation (GDPR). We are not connected to the National Electronic Patient Record system (EPD). Written consent from you is always required before third parties not involved in your treatment may access your records.
An exception applies to health insurance companies. Since July 2010, they may access certain information without your permission.
We would also like to refer you to the Privacy Statement on our website.
Please take care of your personal belongings. We are not liable for any damage, theft, or loss of your property. We advise you not to leave belongings unattended and to bring your coat, bag, and other personal items into the treatment room with you. Use of the coat rack is at your own risk.
We send appointment reminders to our patients by email. No rights can be derived from these reminders; they are intended solely as an additional service.
If, for any reason, you do not receive a reminder from us, this does not mean that your appointment has been cancelled. You remain responsible for attending the appointments you have made.
At your first visit, we will always ask you to complete a questionnaire regarding your health and medication use. It is important that you complete this fully and truthfully in order to prevent complications. Any changes in your health and/or medication should be communicated to us.
We kindly request that smaller amounts (under €150) be paid immediately at the reception desk, either in cash or by debit card. We do not submit invoices directly to health insurance companies; you may submit them yourself.
Unpaid invoices may be transferred to a collection agency, which can result in additional costs. All treatments are subject to the KNMT payment terms and conditions. These payment terms can be viewed via the PDF document.
The payment terms can also be sent to you free of charge upon request. For planned treatments exceeding €250, a treatment estimate will be prepared. By scheduling an appointment based on this estimate, you agree to the proposed costs.
We greatly appreciate suggestions, ideas, comments, and feedback regarding treatments or any other matters.
